You will have something like below in CCcam.cfg, in telnet type dmesg and you will find the exact usb ports your smartreader are connected to.
SERIAL READER : /dev/ttyUSB0 smartreader+
SERIAL READER : /dev/ttyUSB1 smartreader+
SERIAL READER : /dev/ttyUSB2 smartreader+
SERIAL READER : /dev/ttyUSB3 smartreader+
You will also need to overclock these cards, I dont have the exact frequency for each card.

i dont know why not work my 2 card from dm500 s

In dm500 there is no usb port and only 1 internal reader which is /dev/sci0
