Epg is not important. I need example from the playlist. You probably have to turn off static urls. Then regenerate bouquets and then put in favorites. In this way if the stream url changes it will still work from now on.

I found a bug with OpenATV 7.6 & M3UIPTV.
If M3UIPTV manager is ON and if I use vertical EPG it will causing E2 crash. If M3UIPTV manager is OFF vertical EPG works normally.
So I create channel lists and turn manager off after that. It's not a big deal but good to know.

extracted all files in M3UIPTV folder /usr/lib/enigma2/python/Plugins/SystemPlugins/M3UIPTV

When you get the zip from github extract it and from folder src get all files and copy them in the box in directory
/usr/lib/enigma2/python/Plugins/SystemPlugins/M3UIPTV
as your instruction all data extracted to /usr/lib/enigma2/python/Plugins/SystemPlugins/M3UIPTV

Yes that is fixed in latest code. But dont know when it will be in the image itself.
